This would be the same as setting up a hobber as a thread mill- no diff required. The feed gears are set to the lead of the thread and the index gears are calculated to control the chip load.
Dudleys gear handbook has a section on this. You would need a fixture to index the part for each lead, and it would require 4 passes. As Bill mentioned, the tool may not hold up long enough to finish a part though. Normally done on a BC hobber using a right angle hob head.
