Problem with Spiral Bevel Set

Hi All,
I have a CPC-BV1 user that is having trouble cutting a 20x20T Spiral Bevel Set on a Gleason No. 16. Seems like it should not be a problem, but if CPC-BV1 has a problem with miter sets, please let me know. I'm about to pull my hair out trying to see what the problem is.

In the pics he provides, there is a cut or gouge near the topland that shouldn't be there. You can see it at the heel in the photo.
I was guessing a shorter roughing blade mounted in the wrong position in the cutter - I've never seen that situation, so don't know what that would look like.

This is his 2nd or 3rd gearset, but 1st miter, on this machine so it is either something he's missing, or something I'm missing...
